Description
Indications/Uses
Alcoholic liver disease, nonalcoholic steatohepatitis (NASH), hepatitis (viral drug induced), intra hepatic cholestasis of pregnancy, primary biliary cirrhosis, primary sclerosing cholangitis, liver disease in cystic fibrosis; gallstone dissolution and prevention.
Dosage/Direction for Use
Individualized dose. As prescribed by the physician.
Administration
Should be taken with food.
Contraindications
Calcified cholesterol stones, radiopaque stones or radiolucent bile pigment stones, allergy to bile acids, chronic liver disease.
Adverse Reactions
Ursodeoxycholic acid is generally well tolerated. It may cause nausea, vomiting and other gastrointestinal disturbances but diarrhea is reported to occur less frequently than with chenodeoxycholic acid. Increased liver enzyme values are also less likely. Pruritus may occur. Other adverse effect reactions are headache, insomnia, bronchitis, upper respiratory tract infection (URTI), arthralgia, myalgia.
Drug Interactions
Aluminum based antacids, cholestyramine and colestipol, clofibrate, oestrogen, oral contraceptives and perhaps other lipid lowering drugs may counteract the effectiveness of Ursodiol.

Action
Pharmacology: Ursodeoxycholic acid increases the bile acid pool and decreases the hydrophobic bile acid concentration. The biliary flow is increased. It decreases the biliary and serum cholesterol. The cholesterol saturation of the bile is reduced by Ursodiol allowing gradual solubilization of cholesterol gallstones. Cholesterol secretion into the bile is reduced and bile acid concentration rates increases during Ursodiol treatment without a reduction in phospholipids. The rate controlling enzyme of hepatic cholesterol bile synthesis (HMG CoA reductase) hydroxymethyl glutaryl co-enzyme and the catabolic enzyme cholesterol 7α hydroxylase are altered (reduced activity) by Ursodiol therapy.
It is possible that Ursodiol reduces cholesterol and bile salt absorption from the gastrointestinal tract. It has a stabilizing effect on membrane structure and thus exert hepatoprotective action. It also exhibits some immune-modulatory effect.
