This is a prescription-only medicine and a compounded medication. The law prohibits the transfer of this drug to any person other than the patient for whom it was prescribed.

Label: Tirzepatide 5mg Subcutaneous Injection - Lyophilized Powder for Reconstitution in Multi-dose Vial with 5mL Non-bacteriostatic Sterile Water for Injection (SWFI).
Purpose: Compounded Tirzepatide is used for managing type 2 diabetes by improving blood sugar control and for weight management in adults who are overweight or have obesity and at least one weight-related complication.
 
INGREDIENTS: Lyophilized Tirzepatide Powder for Reconstitution
 
HOW TO USE:
Instructions: Administer as directed by your healthcare provider. The medication is typically given as a subcutaneous injection in the abdomen, thigh, or upper arm. Rotate injection sites with each dose to avoid irritation. Do not mix with insulin.
Dosing: Dosing should be directed by your doctor. The standard dosing is provided below:
  • For Type 2 Diabetes: Start with 2.5mg once weekly for 4 weeks, then increase to 5mg. Adjust as needed in 2.5 mg increments (Maximum dose is 15 mg).
  • For Weight Management: Start with 2.5mg once weekly for 4 weeks, then increase to 5mg weekly, adjusting every 4 weeks up to 15mg if necessary.

Sample Dosing

  • 2.5 mg (1mL) each week for weeks 1 to 4 
  • 5mg (2 mL) each week for weeks 5 to 8 
  • 7.5mg (3mL) each week for weeks 9 to 12
  • 10mg (4mL) each week for weeks 13 to 16
  • 12.5mg (5mL) each week for weeks 17 to 20
  • 15mg (6mL) each week for weeks 20 and onwards
Missed Dose: If a dose is missed, administer as soon as possible within 4 days. If more than 4 days have passed, skip the missed dose and resume the regular schedule.
 
EXPECTED RESULTS:
Effectiveness: Tirzepatide enhances insulin secretion, reduces appetite, and helps manage blood glucose and weight.
Onset: Blood sugar improvements may be seen within days, while weight loss can take weeks to months of consistent use.
Clinical Evidence: Clinical trials show significant HbA1c reduction and weight loss in adults with diabetes and obesity.
 
SAFETY & PRECAUTIONS:
Possible Side Effects: Nausea, diarrhea, decreased appetite, vomiting, constipation, and abdominal pain.
Warnings:
  • Thyroid Tumors: Do not use if you or your family has a history of medullary thyroid carcinoma (MTC) or MEN 2. Report any neck mass, swallowing difficulty, or hoarseness.
  • Pancreatitis: Report symptoms of pancreatitis and gallbladder disease.
  • Hypoglycemia: Monitor blood glucose levels closely, especially if using insulin.
  • Vision Changes: Report any vision changes as this could indicate retinopathy.
  • Mental Health Changes: Report any worsening depression or suicidal thoughts.
  • Kidney Health: Monitor for kidney issues, especially if experiencing dehydration.
 
STORAGE:
Conditions: 
  • Unreconstituted Lyophilized Powder is stable at room temperatures of 18ºC to 25ºC (64°F to 77°F) and should not be refrigerated.
  • Reconstituted Lyophilized Powder should be stored in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. 
  • Once in use, Reconstituted Lyophilized Powder must be stored in a refrigerator at 2°C to 8°C (36°F to 46°F).
 
EXPIRY AND BEYOND USE DATES:
  • Expiry as written on the vial (1-2 years)
  • Beyond-Use Date: Reconstituted Lyophilized Powder should only be used 6 weeks from reconstitution.

  • What if I experience severe gastrointestinal symptoms?

RECONSTITUTION AND ADMINISTRATION INSTRUCTIONS LEAFLET

 
This is a prescription-only medicine and a compounded medication. The law prohibits the transfer of this drug to any person other than the patient for whom it was prescribed. 
 
Keep Out of Reach of Children.
 
WHAT’S IN THE BOX: Your package should include:
  1. One (1) Multi-dose Vial of Tirzepatide 5mg Subcutaneous Injection - Lyophilized Powder for Reconstitution
  2. One (1) 5mL Non-bacteriostatic Sterile Water for Injection (SWFI) ampule
  3. One (1) 3mL Syringe NOTE: for reconstitution only
  4. Three (3) Alcohol Swabs
  5. Three (3) 1mL Syringes (“Insulin Syringes”)
 
RECONSTITUTION INSTRUCTIONS:
  1. Clean your hands thoroughly and set up all components on a clean, organized surface. 
  2. Wipe down any surfaces to eliminate potential contamination and prepare a designated sharps container for used needles.
  3. Examine all components, including multi-dose vials and syringes, to ensure they are intact, of good quality, and free of defects.
  4. Carefully twist open the cap of the 5mL non-bacteriostatic SWFI ampule and set it aside in an upright position.
  5. Remove the cap from the multi-dose vial to expose the rubber top and set it upright.
  6. Use an alcohol swab to wipe and disinfect the rubber top of the multi-dose vial and place the used swab on its original packaging for disposal.
  7. Using the 3mL syringe, draw 2mL of the non-bacteriostatic SWFI from the ampule.
  8. With one hand, hold the filled syringe steady, and with the other hand, insert the needle into the rubber top of the multi-dose vial.
  9. Slowly push the plunger to introduce the non-bacteriostatic SWFI into the vial, then mix the vial until a clear solution is achieved, removing the syringe once mixing is complete.
  10. Dispose of the used needle immediately in the designated sharps container.
 
AVOIDING NEEDLE-STICK INJURY:
  1. Use a one-handed technique when recapping used needles.
  2. Keep your workspace clutter-free to prevent accidents.
  3. Remove any potential sources of distraction or disturbance.
  4. You may use gloves and protective gear when handling needles.
  5. Report any needle-stick incidents immediately to a healthcare provider.
 
ADMINISTRATION INSTRUCTIONS:
  1. Clean your hands and prepare a clean, organized surface with the reconstituted multi-dose vial, a 1mL syringe, and an alcohol swab.
  2. Wipe down surfaces to eliminate contamination and prepare a designated sharps container.
  3. Inspect all components to ensure they are intact and free of dirt or defects.
  4. Shake the reconstituted multi-dose vial to ensure the solution is uniform.
  5. Use an alcohol swab to disinfect the rubber top of the vial and dispose of the swab properly.
  6. While the syringe is empty, insert the syringe into the vial then draw the prescribed dose.
  7. Set the vial aside after filling the syringe, ensuring you measure accurately (i.e., use the top part of the plunger seal to measure the appropriate volume).
  8. For injection, pinch the skin and insert the needle at a 45º angle into the abdomen, thigh, or upper arm, rotating sites to prevent irritation.
  9. Inject the medication slowly and leave the needle in for 5 seconds after injecting.
  10. Remove and dispose of the needle in the sharps container.
