Administration
Special Precautions
Salbutamol should be given with caution in hyperthyroidism, myocardial insufficiency, arrhythmias, susceptibility to QT-interval prolongation, hypertension and diabetes mellitus.
In severe asthma, particular precaution is required to avoid inducing hypokalemia as this effect may be potentiated by hypoxia or by concomitant administration of other antiasthma drugs. Plasma potassium concentrations should be monitored.
Use In Pregnancy & Lactation
The use of salbutamol is not recommended in nursing mothers unless the expected benefit is greater than any possible risks.
Adverse Reactions
Very rarely, hypersensitivity reactions eg, bronchospasm, urticaria, angioedema, hypotension and collapse have been reported with salbutamol.
Side effects eg, GI discomfort, nausea and vomiting have occasionally been reported with guaifenesin, particularly in very large doses.
Storage
Shelf-Life: 36 months.
Action
On the other hand, salbutamol is a direct-acting sympathomimetic agent with predominantly β-adrenergic activity and selective action on β2-receptors. It is used as a bronchodilator. It has more prolonged actions than isoprenaline and as a predominantly β2-stimulant, has a bronchodilating action relatively more prominent than its effect on the heart. An additional beneficial effect of salbutamol is its action on the uterus as a relaxant. The uterine muscles contract under β-adrenergic stimulation. Thus, it has gained popularity among obstetricians for controlling premature labor. However, a relative contraindication to its use is among pregnant patients with vaginal bleeding because it may cause pulmonary edema.
