PREXIA Esomeprazole 40mg is used for the treatment of conditions caused by excessive stomach acid, such as gastroesophageal reflux disease (GERD) and the healing of erosive esophagitis (acid-related damage to the esophagus). It is also used to prevent gastric ulcers associated with the use of nonsteroidal anti-inflammatory drugs (NSAIDs) and for the eradication of Helicobacter pylori (H. pylori) bacteria, usually in combination with antibiotics. Furthermore, it is indicated for the treatment of pathological hypersecretory conditions, including Zollinger-Ellison syndrome.

Side Effects
PREXIA Esomeprazole 40mg common side effects include headache and various gastrointestinal disturbances such as diarrhea, nausea, abdominal pain, flatulence, constipation, and dry mouth. Less common effects can involve drowsiness or a bad taste in the mouth.Prolonged use, particularly for over a year, may be associated with more serious issues, including the development of very low blood magnesium levels (hypomagnesemia), an increased risk of bone fractures in the hip, wrist, or spine, and potential Vitamin B12 deficiency due to reduced absorption. Rare but severe adverse reactions such as severe diarrhea, kidney problems, or symptoms of lupus may also occur.
Directions and Dosage
- Dosage for Adults (18+ years) with Erosive Esophagitis: 20 mg or 40 mg once daily. Treatment duration is typically 4 to 8 weeks. For maintenance of healing, 20 mg once daily may be used for up to 6 months.
- Dosage for Adults (18+ years) for Symptomatic GERD: 20 mg once daily for 4 weeks.
- Dosage for Adults (18+ years) for H. pylori eradication: 40 mg once daily for 10 days, combined with appropriate antibiotics.
- Dosage for Adults (18+ years) for Risk Reduction of NSAID-Associated Gastric Ulcer: 20 mg or 40 mg once daily for up to 6 months.
- Dosage for Adults (18+ years) for Zollinger-Ellison Syndrome: Initial dosage is 40 mg twice daily. The dosage is adjusted as needed, with daily doses up to 240 mg reported. Doses above 80 mg daily should be divided and given twice-daily.
- Dosage for Adolescents (12-17 years) with Erosive Esophagitis: 20 mg or 40 mg once daily for 4 to 8 weeks.
- Dosage for Kids 1-11 years (weighing $\geq$ 20 kg) with Erosive Esophagitis: 10 mg or 20 mg once daily for 8 weeks.
- Dosage for Infants 1 month - $\lt$ 1 year (weighing $\gt$ 7.5 kg - 12 kg) with Erosive Esophagitis: 10 mg once daily for up to 6 weeks.
Not recommended for children under 1 year old for symptomatic reflux or children under 1 month old for erosive esophagitis. The maximum daily dosage varies significantly based on the condition being treated, but for standard GERD/Esophagitis, 40 mg once daily is a common maximum for adults.
Contraindications
This medication should not be used if there is a known hypersensitivity to the active substance, other substituted benzimidazoles (the class of compounds it belongs to), or any of the product's components. Concurrent use with certain specific antiviral medications, such as nelfinavir or rilpivirine, is also not recommended due to serious interaction risks.
Special Precautions
- Before starting treatment, the presence of a gastric malignancy should be excluded, as symptomatic response to treatment may delay diagnosis.
- Long-term or high-dose therapy, particularly for over a year, has been associated with an increased risk of bone fractures in the hip, wrist, or spine.
- Extended use may lead to very low blood magnesium levels (hypomagnesemia), which can cause fatigue, confusion, seizures, or irregular heart rhythm.
- Using it for a prolonged period may result in reduced absorption of Vitamin B12, which should be monitored for deficiency.
- Treatment may be associated with an increased risk for certain infections, including Clostridium difficile-associated diarrhea.
- Rarely, acute tubulointerstitial nephritis, a type of kidney problem, or skin reactions like cutaneous lupus erythematosus have been reported.
Is it safe to take it with other medications?
Caution is advised as this medicine can alter the absorption of other compounds whose effectiveness relies on stomach acidity, such as certain antifungals and iron salts. It can also increase the blood levels of certain compounds metabolized in the liver, like diazepam and phenytoin. Combining it with specific HIV medications or methotrexate requires careful consideration.
How should I store it?
The capsules should be stored at room temperature, ideally below 30°C. They should be kept in the original container to protect them from moisture and heat.
