Indications/Uses
To clean the bowel prior to x-ray exam or endoscopy. To clean the bowel prior to surgery when judged clinically necessary.
Dosage/Direction for Use
Adult including the elderly Split-dose regimen (evening before & day of the procedure) 1st sachet is taken the night before the procedure & 2nd is taken the next day, in the morning prior to the procedure. On the day before the procedure: 1st reconstituted sachet is taken in the evening (eg, 5:00-9:00 pm) followed by at least five 250-mL drinks of clear liqd. On the day of the procedure: 2nd reconstituted sachet is taken in the morning (5-9 hr before the procedure) followed by at least three 250-mL drinks of clear liqd. Clear liqd may be consumed until 1 hr before the procedure. Day-before regimen (evening before the procedure only) 1st sachet is taken in the afternoon or early evening & 2nd is taken approx 6 hr later, the night before the procedure. On the day before the procedure: 1st reconstituted sachet is taken in the afternoon or early evening (eg, 4:00-6:00 pm) followed by at least five 250-mL drinks of clear liqd. 2nd reconstituted sachet is taken in the late evening (eg, 10:00 pm-12:00 am) followed by at least three 250-mL drinks of clear liqd consumed until 1 hr before the procedure. Childn ≥9 yr Adult dose, 4-9 yr 1 sachet morning, ½ sachet afternoon, 2-4 yr ½ sachet morning & afternoon, 1-2 yr ¼ sachet morning & afternoon. To be taken before 8:00 am on the day before the procedure followed by 2nd dose 6-8 hr later.
Administration
Should be taken on an empty stomach.
Contraindications
Hypersensitivity. CHF, gastric retention, GI ulceration, toxic colitis, toxic megacolon, ileus, nausea & vomiting, acute surgical abdominal conditions (eg, acute appendicitis), known or suspected GI obstruction or perforation, severe dehydration, rhabdomyolysis, hypermagnesemia, active inflammatory bowel disease, severe renal impairment.
Special Precautions
Bowel surgery. Patients w/ recent GI surgery, renal impairment, heart or inflammatory bowel disease. Concomitant use w/ drugs that might affect water &/or electrolyte balance eg, diuretics, corticosteroids & lithium. Ensure adequate oral intake of water & electrolytes particularly in less fit patients, childn, elderly, debilitated & those at risk of hypokalemia. Patients on antiepileptics, w/ previously controlled epilepsy; reduced kidney function or those on a controlled K diet. Period of bowel cleansing should not be >24 hr. Rare hereditary problems of galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Not to be used as a routine laxative. Pregnancy & lactation.
Adverse Reactions
Headache, nausea & proctalgia.
Drug Interactions
May affect absorption of antiepileptics, contraceptives, antidiabetics & antibiotics. Tetracycline & fluoroquinolone, Fe, digoxin, chlorpromazine & penicillamine (take at least 2 hr before & not <6 hr after administration of Picoprep to avoid chelation w/ Mg). Reduced efficacy w/ bulk-forming laxatives. Increased risk of hypokalemia w/ diuretics, corticosteroids or cardiac glycosides; water retention &/or electrolyte imbalance w/ NSAIDs, drugs known to induce SIADH eg, TCAs, SSRIs, antipsychotic drugs & carbamazepine.
