Indications / Uses

TEGRETOL SR Carbamazepine 200mg is used for the management of various seizure types in epilepsy, including complex partial, simple partial, and generalized tonic-clonic seizures. The sustained-release formulation is designed to maintain consistent levels of the compound for extended periods. It is also indicated as a primary treatment for the intense facial nerve pain associated with trigeminal neuralgia, as well as idiopathic glossopharyngeal neuralgia. Furthermore, this medication helps manage acute mania and serves as a maintenance treatment for bipolar affective disorders to help prevent or reduce the recurrence of mood episodes.

Side Effects

TEGRETOL SR Carbamazepine 200mg common side effects include dizziness, general drowsiness, problems with coordination or unsteadiness, and nausea or vomiting. Visual disturbances like blurred or double vision may also occur. More serious, but rare, effects involve life-threatening severe skin reactions, such as Stevens-Johnson syndrome, which can present as a spreading rash or blisters. There is also a risk of a significant reduction in the body’s blood cell count, signaled by symptoms like fever, persistent sore throat, or unusual bruising.

Directions and Dosage

• Dosage for Adults (18+ years): Initial dose is 200 mg two times daily. The dose is then increased gradually until an optimal response is obtained, usually reaching 800 mg to 1200 mg daily. For epilepsy and bipolar disorder, the maximum recommended dosage is 1600 mg in 24 hours. For trigeminal neuralgia, the maximum dosage is 1200 mg in 24 hours.
• Dosage for Adolescents 12–18 years: Initial dose is 200 mg two times daily. Do not exceed 1200 mg in 24 hours.
• Dosage for Kids 6–12 years: Initial dose is 100 mg two times daily. Do not exceed 1000 mg in 24 hours.

Not recommended for children under 6 years without body weight-based dosage determination. The maximum daily dosage for any child is generally 35 mg per kg of body weight in 24 hours.

Contraindications

Use is not suitable for individuals with a known severe sensitivity to the compound or related chemical structures, such as tricyclic antidepressants. It must not be used in patients with a history of bone marrow depression, certain heart conduction defects, or a history of hepatic porphyrias. It is also contraindicated if certain antidepressants, specifically MAO inhibitors, have been taken within the last 14 days.

Use in Pregnancy and Lactation

Use during pregnancy carries a known risk of congenital malformations in the developing fetus, including spina bifida. The compound passes into milk and may affect the infant. Furthermore, the effectiveness of hormonal birth control methods, such as oral pills or implants, may be significantly decreased during treatment.

Special Precautions

• Monitor for signs of severe skin reactions like a spreading rash, blisters, or ulcers, especially during the first few months of use.
• Discontinue use immediately if there is any evidence of severe blood-related issues, such as persistent fever, sore throat, or unusual bruising.
• Periodic monitoring of blood counts and liver function is advised during prolonged treatment.
• Patients with a history of heart, liver, or kidney problems should be closely observed.
• Avoid sudden discontinuation of the treatment, as this can increase the risk of worsening symptoms.

Is it safe to take it with other medications?

This compound can affect the levels of many co-administered medications by speeding up their breakdown in the body, potentially rendering them less effective. This includes essential medications like hormonal contraceptives, which may fail to prevent pregnancy. It should not be used in combination with MAO inhibitors or certain other compounds.
