Contraindications
The use of this medication is not appropriate for individuals with a known severe sensitivity to the active compound or other chemically related antidepressants. It is also contraindicated in those with a history of bone marrow depression, certain types of heart block, or hepatic porphyrias. Use is not permitted for patients who have recently taken or are currently taking certain types of antidepressants known as MAO inhibitors, or the medication nefazodone.
Use in Pregnancy and Lactation
The compound is associated with specific risks during pregnancy and may cause congenital malformations such as spina bifida and developmental delays. It has also been shown to decrease the effectiveness of hormonal contraceptives, including pills and implants. The compound passes into milk.
Special Precautions
- Monitor for any signs of skin reactions, especially a painful rash, blistering, or mouth sores, particularly during the initial months of use.
- The compound may lead to a decrease in the number of blood cells. Be aware of symptoms like unexplained fever, sore throat, or unusual bruising.
- Patients with a history of heart issues, particularly conduction problems, or pre-existing liver disease require careful attention during treatment.
- Discontinuation of the medication must be done gradually to avoid increasing the risk of seizures.
Is it safe to take it with other medications?
The compound interacts with many other medications by altering their metabolism, which can reduce the effectiveness of co-administered drugs. This is particularly important for hormonal contraceptives, where reduced efficacy is a known risk. Concomitant use with MAO inhibitors is not permitted, and the effects of alcohol or other central nervous system depressants may be intensified.
