Indications / Uses

ZEMDIL Diltiazem 30mg is used for the management of hypertension, either alone or in combination with other blood pressure treatments. As a calcium channel blocker, this medicine helps improve exercise tolerance and manage chronic stable angina, commonly known as classic effort-associated chest pain. The medication is also indicated for angina pectoris that is caused by coronary artery spasm, including Prinzmetal's variant angina. This active substance works by relaxing blood vessels and increasing the supply of blood and oxygen to the heart.

Side Effects

ZEMDIL Diltiazem 30mg common side effects include headache, dizziness, and peripheral edema, typically presenting as swelling in the ankles or feet. Other frequently reported effects involve the gastrointestinal system, such as constipation and nausea, as well as facial flushing and a temporary reduction in heart rate. While rare, more severe effects can include cardiac conduction disturbances, dangerously low blood pressure, or signs of liver distress like jaundice or dark urine, all of which require immediate attention.

Directions and Dosage

• Dosage for Adults: The initial dose for conventional tablets is typically 30 mg taken 4 times daily.
• Dosage may be gradually increased every 1 to 2 days until the desired therapeutic effect is achieved.
• The usual maintenance dosage range is 180 mg to 360 mg daily, divided into 3 to 4 separate doses.
• Do not exceed a maximum of 360 mg in 24 hours.

Use in patients under 18 years of age is not recommended due to limited established data on safety and efficacy in these age groups.

Contraindications

The medicine should not be used in individuals with a known allergy to the active component. It is strictly restricted in patients who have certain serious pre-existing heart rhythm conditions, such as second- or third-degree atrioventricular block, unless a functional ventricular pacing device is installed. Use is also prohibited in cases of severe low blood pressure or an acute heart condition accompanied by fluid accumulation in the lungs.

Use in Pregnancy and Lactation

Studies in animals have indicated potential adverse effects on the fetus. Therefore, the use of this drug during gestation should be avoided unless the expected benefits significantly outweigh the potential risk. The active substance passes into breast milk, and use during nursing should be approached with caution.

Special Precautions

• Exercise special caution if there is pre-existing impairment of kidney or liver function, as dosage adjustment may be needed.
• The medication should be used carefully in individuals with pre-existing cardiac failure or abnormally slow heart rates.
• Monitoring for cardiac conduction disturbances is necessary, which may rarely lead to abnormally slow heart rates or heart block, especially in susceptible individuals.

Is it safe to take it with other medications?

Combining this treatment with certain other heart rate-lowering agents or blood pressure medicines can cause an additive effect, potentially requiring dosage changes of one or both treatments. Specific contraindications exist with certain strong metabolic enzyme inhibitors, such as lomitapide, due to increased risk of toxicity and severe cardiovascular collapse.

How should I store it?

Keep the tablets at a temperature below 30°C and protect them from direct heat, moisture, and sunlight. Ensure the medicine is kept securely away from the reach of children.
