Description

Indications/Uses

Essential HTN & initial therapy in patients who are likely to need multiple drugs to achieve BP goals.
 

Dosage/Direction for Use

Initially 5/80 mg once daily, may be increased to a max of one 10/320 mg tab once daily after 1-2 wk.
 

Administration

May be taken with or without food.
 

Contraindications

Hypersensitivity. Angiotensin receptor antagonists or ACE inhibitors w/ aliskiren in patients w/ type 2 DM. Pregnancy.
 

Special Precautions

Symptomatic hypotension may occur in patients w/ Na &/or vol depletion. Discontinue immediately if angioedema occur. Hyperkalemia. Frequent monitoring of K. Patients w/ unilateral or bilateral renal artery stenosis, serious CHF (NYHA III-IV), acute MI, aortic or mitral valve stenosis or obstructive hypertrophic cardiomyopathy. Kidney transplantation. Renal & hepatic impairment. Not to be used in women planning to become pregnant. Not advisable during pregnancy & lactation. Childn <18 yr.
 

Adverse Reactions

Nasopharyngitis, flu; headache; edema, pitting edema, facial edema, edema peripheral, fatigue, flushing, asthenia, hot flush.
 

Drug Interactions

Amlodipine: Increased systemic exposure w/ simvastatin. Increased plasma conc w/ strong CYP3A4 inhibiors eg, ketoconazole, itraconazole, ritonavir. Valsartan: Increased incidence of hypotensive, hyperkalemia & changes in renal function w/ dual blockade of renin-angiotensin-system (RAS) w/ ARBs, ACE inhibitors or aliskiren. Increased K levels (heparin). Increased risk of worsening of renal function w/ angiotensin II antagonists & NSAIDs. Increased serum lithium conc. Increased systemic exposure w/ inhibitors of uptake transporter (rifampin, ciclosporin) or efflux transporter (ritonavir).
