Side Effects
SORBANCE-5 Isosorbide Dinitrate 5mg common side effects include: headache, which is frequently reported and may be severe, and dizziness, especially when standing up too quickly (postural hypotension). Other effects may include flushing of the face and neck, nausea, or vomiting. More serious but less common reactions can involve a significant drop in blood pressure, an increase in heart rate (reflex tachycardia), and signs of methemoglobinemia, which presents as bluish discoloration of the lips and skin.
Directions and Dosage
This product is available in two forms for angina: one for quick action (sublingual) and one for long-term prevention (oral).
- Dosage for Adults (Oral Tablet for Angina Prevention): Initial dose is 5 mg to 20 mg, 2 or 3 times daily.
- Maintenance Dosage for Adults (Oral Tablet for Angina Prevention): 10 mg to 40 mg, 2 or 3 times daily.
- Dosage for Adults (Sublingual Tablet for Acute Angina or Prophylaxis before Stress): 2.5 mg to 5 mg as needed.
- Dosage for Adults (Oral Tablet for Congestive Heart Failure): 40 mg to 160 mg daily, divided into smaller doses. Do not exceed 240 mg in 24 hours.
A daily dose-free interval is often advised to minimize tolerance to the medication's effects. Safety and effectiveness in children have not been established.
Contraindications
The product should not be administered to individuals with known hypersensitivity or allergy to nitrates or related compounds. It is also contraindicated in patients taking phosphodiesterase type 5 (PDE5) inhibitors (like sildenafil) or a soluble guanylate cyclase (GC) stimulator (like riociguat), as these combinations can lead to dangerously severe hypotension. Avoid use in cases of severe anemia, shock, circulatory collapse, or certain heart conditions like obstructive cardiomyopathy.
Use in Pregnancy and Lactation
Use during pregnancy is generally not recommended unless the benefit is considered necessary. Safety has not been definitively established, with animal studies showing potential risk at high doses. It is unknown if the substance passes into breast milk, so caution should be exercised if administered to a woman who is nursing.
Special Precautions
