Indications / Uses

SORBANCE-5 Isosorbide Dinitrate 5mg is used for the long-term treatment and prevention of angina pectoris (chest pain) associated with coronary artery disease. It is also used as part of the long-term management of severe chronic heart failure. This medication works as a vasodilator, helping to relax and widen blood vessels to improve blood flow and reduce the heart's workload.

Side Effects

SORBANCE-5 Isosorbide Dinitrate 5mg common side effects include: headache, which is frequently reported and may be severe, and dizziness, especially when standing up too quickly (postural hypotension). Other effects may include flushing of the face and neck, nausea, or vomiting. More serious but less common reactions can involve a significant drop in blood pressure, an increase in heart rate (reflex tachycardia), and signs of methemoglobinemia, which presents as bluish discoloration of the lips and skin.

Directions and Dosage

This product is available in two forms for angina: one for quick action (sublingual) and one for long-term prevention (oral).

• Dosage for Adults (Oral Tablet for Angina Prevention): Initial dose is 5 mg to 20 mg, 2 or 3 times daily.
• Maintenance Dosage for Adults (Oral Tablet for Angina Prevention): 10 mg to 40 mg, 2 or 3 times daily.
• Dosage for Adults (Sublingual Tablet for Acute Angina or Prophylaxis before Stress): 2.5 mg to 5 mg as needed.
• Dosage for Adults (Oral Tablet for Congestive Heart Failure): 40 mg to 160 mg daily, divided into smaller doses. Do not exceed 240 mg in 24 hours.

A daily dose-free interval is often advised to minimize tolerance to the medication's effects. Safety and effectiveness in children have not been established.

Contraindications

The product should not be administered to individuals with known hypersensitivity or allergy to nitrates or related compounds. It is also contraindicated in patients taking phosphodiesterase type 5 (PDE5) inhibitors (like sildenafil) or a soluble guanylate cyclase (GC) stimulator (like riociguat), as these combinations can lead to dangerously severe hypotension. Avoid use in cases of severe anemia, shock, circulatory collapse, or certain heart conditions like obstructive cardiomyopathy.

Use in Pregnancy and Lactation

Use during pregnancy is generally not recommended unless the benefit is considered necessary. Safety has not been definitively established, with animal studies showing potential risk at high doses. It is unknown if the substance passes into breast milk, so caution should be exercised if administered to a woman who is nursing.

Special Precautions

• Use with caution in patients who may be volume depleted (e.g., from diuretic therapy) or already have low blood pressure.
• Severe hypotension, particularly when changing position from sitting or lying down, may occur. Change position slowly.
• May **aggravate angina** in patients with hypertrophic cardiomyopathy.
• Patients on long-term therapy may develop tolerance to the effects, making a dose-free interval necessary to restore effectiveness.

Is it safe to take it with other medications?

Concomitant use with drugs for erectile dysfunction*(e.g., sildenafil, tadalafil) or certain pulmonary hypertension treatments is strictly contraindicated due to the risk of a dangerous drop in blood pressure. Combining with other antihypertensive drugs or vasodilators should be done with caution, as this may enhance hypotensive effects.
