Indications / Uses

NOVOPTIN Diltiazem 60mg is used for the long-term management of chronic stable angina, addressing chest pain related to physical effort. This medicine is also indicated for angina pectoris resulting from coronary artery spasm, including the variant known as Prinzmetal's angina. As a calcium channel blocker, it works by relaxing the smooth muscles of the coronary arteries, promoting vasodilation. It is further employed in the treatment of hypertension, either alone or in combination with other blood pressure agents.

Side Effects

NOVOPTIN Diltiazem 60mg common side effects include headache, fatigue, and peripheral edema, often observed as swelling in the lower extremities like the ankles or feet. Gastrointestinal issues such as constipation, nausea, and dyspepsia are also common occurrences. Additionally, some individuals may experience facial flushing and a temporary slowing of the heart rate. Rare but serious effects involve heart conduction abnormalities, severe drops in blood pressure, or liver dysfunction signs, which require immediate attention.

Directions and Dosage

• Dosage for Adults: The usual starting dosage for conventional tablets is 30 mg or 60 mg, administered 4 times daily.
• Dosage adjustment, if necessary, should be gradual, occurring every 1 to 2 days to achieve the optimal therapeutic effect.
• The standard maintenance dosage range is 180 mg to 360 mg daily, divided into 3 to 4 equal doses.
• Do not exceed a maximum of 360 mg in 24 hours.

The safety and effectiveness of this medication have not been established in patients under 18 years of age, and its use is therefore not advised for younger individuals.

Contraindications

The medicine should not be used in individuals with a known hypersensitivity to the active component or other contents. It is strictly restricted in patients who have certain serious pre-existing heart conduction issues, such as second- or third-degree atrioventricular block, unless a functional ventricular pacing device is installed. Use is also restricted in cases of very low blood pressure or acute heart conditions accompanied by fluid build-up in the lungs.

Use in Pregnancy and Lactation

Animal studies have indicated potential adverse effects on the developing fetus. Therefore, the use of this drug during gestation should be avoided unless the anticipated benefits significantly outweigh the potential risk. The active substance is known to pass into breast milk, and use during nursing should be carried out with special caution.

Special Precautions

• Exercise special caution if there is pre-existing impairment of kidney or liver function, as dosage adjustment may be needed.
• The medication should be used carefully in individuals with pre-existing heart failure or abnormally slow heart rates.
• Monitoring for cardiac conduction disturbances is necessary, which may rarely lead to abnormally slow heart rates or heart block, especially in susceptible individuals.

Is it safe to take it with other medications?

Combining this treatment with certain other heart rate-lowering agents or blood pressure treatments can cause an additive effect, potentially requiring dosage changes of one or both therapies. Specific restrictions exist with certain strong metabolic enzyme inhibitors, such as lomitapide, due to the increased risk of toxicity and severe cardiovascular collapse.

Keep the tablets at a temperature below 30°C and protect them from direct heat, moisture, and sunlight. Ensure the medicine is kept securely away from the reach of children.
