Indications / Uses

ENTRESTO Sacubitril - Valsartan 50mg is used for the treatment of symptomatic chronic heart failure in adult patients with reduced ejection fraction to reduce the risk of cardiovascular death and heart failure hospitalization. It is also indicated for the treatment of symptomatic heart failure in pediatric patients aged 1 year and older. This combination medication works by enhancing the protective neurohormonal systems of the heart while simultaneously inhibiting the harmful effects of the overactive renin-angiotensin-aldosterone system.

Side Effects

ENTRESTO Sacubitril - Valsartan 50mg common side effects include: low blood pressure (hypotension), high potassium levels (hyperkalemia), and decreased kidney function (renal impairment). Some patients may also experience cough, dizziness, or angioedema. Allergic reactions involving swelling of the face, lips, tongue, or throat are serious but less frequent.

Directions and Dosage

  • Adults (18+ years): Starting dose is 100mg (one 50mg tablet twice daily). Target maintenance dose is 200mg (one 100mg tablet twice daily) after 2–4 weeks.
  • Pediatrics (1 year and older) weighing <40 kg: 0.8mg/kg twice daily; titrate up to 2.3mg/kg twice daily.
  • Pediatrics (1 year and older) weighing 40kg to <50 kg: Starting dose 50mg twice daily; target dose 150mg twice daily.
  • Pediatrics (1 year and older) weighing >50 kg: Starting dose 50mg twice daily; target dose 200mg twice daily.

Not recommended for children under 1 year of age. Maximum daily dosage for adults is 400mg (200mg twice daily). Treatment is usually long-term for chronic conditions.

Contraindications

Hypersensitivity to any component. History of angioedema related to previous ACE inhibitor or ARB therapy. Concurrent use with ACE inhibitors (must wait 36 hours after stopping). Concomitant use with aliskiren in patients with diabetes. Severe hepatic impairment or biliary cirrhosis.

Use in Pregnancy and Lactation

Use during the second and third trimesters of pregnancy can cause injury and even death to the developing fetus. Use is not recommended while breastfeeding as it is unknown if the components pass into breast milk.

Special Precautions

  • Monitor blood pressure and kidney function regularly during treatment.
  • Monitor serum potassium levels, especially in patients at risk for hyperkalemia.
  • Caution is required in patients with severe renal impairment or renal artery stenosis.

Is it safe to take it with other medications?

Avoid use with ACE inhibitors or aliskiren. Caution is advised when used with potassium-sparing diuretics, potassium supplements, or non-steroidal anti-inflammatory drugs (NSAIDs) which may impact kidney function.

How should I store it?

Store in the original package to protect from moisture. Keep the tablets at room temperature not exceeding 30°C.
