Indications / Uses

Irbesartan 150mg is used to treat high blood pressure (essential hypertension) and to protect the kidneys in patients with high blood pressure, type 2 diabetes, and impaired kidney function. As an angiotensin-II receptor antagonist, it works by blocking the action of angiotensin-II, which prevents blood vessels from constricting and helps them relax, lowering blood pressure and slowing kidney function decline.

Side-effects

This medicine can cause side effects, though not everyone will experience them.

Common side effects of Irbesartan 150mg:

Increased potassium levels in blood tests for people with high blood pressure, type 2 diabetes, and kidney disease.
Dizziness
Nausea/vomiting
Fatigue
Elevated creatine kinase enzyme levels (indicating muscle and heart activity)
For patients with high blood pressure, type 2 diabetes, and kidney disease:
- Dizziness and low blood pressure when standing up
- Joint or muscle pain
- Decreased haemoglobin levels in blood tests.

Some side effects may be serious and require immediate medical attention. If you experience any of the following, stop taking the medicine and contact your doctor or go to the nearest hospital:

Not known (frequency cannot be estimated):

Serious allergic reaction: Itchy red rash, dizziness or faintness, swelling of the face, lips, and/or tongue, which may cause difficulty breathing.
Liver problems: Yellowing of the skin and eyes, severe upper stomach pain, nausea, and vomiting.
Vasculitis: Rash with raised purple spots and flat red areas due to inflammation of small blood vessels.
Kidney problems: Changes in urination, including more frequent, pale urine or difficulty passing urine, or dark or bloody urine.

Dosage / Direction for Use

Adults

For patients with high blood pressure:

The recommended starting dose is 150 mg once daily. This can be increased to 300 mg once daily, depending on the blood pressure response.

For patients with high blood pressure and type 2 diabetes with kidney disease:

The recommended maintenance dose is 300 mg once daily for treating the associated kidney disease.

A lower dose may be advised, especially when starting treatment, for those on haemodialysis or aged 75 or older.

The maximal blood pressure-lowering effect is typically reached 4-6 weeks after starting treatment.

Children and Adolescents

Irbesartan should not be given to children under 18 years of age. If a child swallows any tablets, contact your doctor immediately.

How to take?

Irbesartan is for oral use. Swallow the tablets with a sufficient amount of fluid, such as one glass of water. It can be taken with or without food. For best results, try to take your daily dose at the same time each day. It is important to continue taking Irbesartan until your doctor advises you to stop.

Contraindications

Do not take Irbesartan if you:

Are allergic to irbesartan or any of the other ingredients in this medicine.
Are more than 3 months pregnant (It is also advised to avoid Irbesartan in early pregnancy).
Have diabetes or impaired kidney function and are being treated with a blood pressure-lowering medicine that contains aliskiren.

Special Precautions

Talk to your doctor or pharmacist before taking Irbesartan if you:

Have had, or during treatment develop, excessive vomiting or diarrhea.
Have been taking "water tablets" (diuretics) or are on a low-sodium diet.
Have kidney problems, including narrowing of the kidney artery or have had a kidney transplant.
Have heart problems, such as narrowing of the heart valves, an enlarged heart muscle, or heart failure.
Have narrowing of the heart blood vessels, which may cause chest pain (e.g., angina).
Are receiving Irbesartan for diabetic kidney disease. Your doctor may perform regular blood tests, especially for blood potassium levels if kidney function is poor.
Develop low blood sugar levels, particularly if you are being treated for diabetes (symptoms include sweating, weakness, hunger, dizziness, trembling, headache, paleness, numbness, and fast heart rate).
Have primary aldosteronism (a condition with high aldosterone levels, leading to sodium retention and high blood pressure).
Are of Afro-Caribbean origin, as Irbesartan may be less effective at lowering your blood pressure.
Are taking other high blood pressure medications, such as:
- ACE inhibitors (e.g., enalapril, lisinopril, ramipril), particularly if you have diabetes-related kidney issues.
- Aliskiren.

Your doctor may monitor your kidney function, blood pressure, and electrolyte levels (e.g., potassium) regularly.

During treatment, you should also tell your doctor if you are going to have an operation or be given anesthetics.

Children & Adolescent

This medicine should not be used in children and adolescents because its safety and efficacy have not yet been fully established.

Pregnancy and breast-feeding

Pregnancy: You must tell your doctor if you think you are, or might become, pregnant. Your doctor will usually advise you to stop taking Irbesartan before you become pregnant or as soon as you find out you are pregnant and will suggest an alternative medicine. Irbesartan is not recommended during early pregnancy and should not be taken after the third month of pregnancy, as it may cause serious harm to your baby.

Breast-feeding: Tell your doctor if you are breast-feeding or planning to start. Irbesartan is not recommended for mothers who are breast-feeding, and your doctor may choose a different treatment if you wish to breast-feed, especially if your baby is newborn or was born prematurely.

Driving and using machines

Irbesartan generally doesn't affect your ability to drive or use machines. However, if you experience dizziness or fatigue while being treated for high blood pressure, consult your doctor before driving or operating machinery.

Is it safe to take it with other drugs?

If you're taking or planning to take other medicines, inform your doctor or pharmacist, as they may need to adjust your dose or take precautions. This is especially important if you are using:

ACE-inhibitors or aliskiren
Potassium supplements, salt substitutes with potassium, potassium-sparing medications, or medicines like heparin that can raise potassium levels
Lithium (for mental health)
Repaglinide (for lowering blood sugar)

Also, non-steroidal anti-inflammatory drugs (NSAIDs) or "COX-2" inhibitors (e.g., celecoxib, aspirin) may reduce the effectiveness of irbesartan. Regular blood checks may be necessary.

How should I store it?

Store below 30°C. Keep this medicine out of the sight and reach of children.

Dosage / Direksyon sa Paggamit

Mga Matatanda

Para sa mga pasyente na may mataas na presyon ng dugo:
Ang inirerekomendang simula na dosis ay 150 mg isang beses araw-araw. Maaari itong itaas sa 300 mg isang beses araw-araw, depende sa tugon ng presyon ng dugo.

Para sa mga pasyente na may mataas na presyon ng dugo at type 2 diabetes na may sakit sa kidney:
Ang inirerekomendang maintenance dose ay 300 mg isang beses araw-araw para sa paggamot ng kaugnay na sakit sa kidney.

Ang mas mababang dosis ay maaaring irekomenda, lalo na kapag nagsisimula ng paggamot, para sa mga pasyenteng nasa haemodialysis o may edad na 75 o higit pa.

Paano Inumin?

Kontraindikasyon

Huwag inumin ang Irbesartan kung ikaw ay:

Nagbubuntis ng higit sa 3 buwan (Inirerekomenda din na iwasan ang Irbesartan sa unang tatlong buwan ng pagbubuntis).
May diabetes o problema sa kidney at tinatrato gamit ang gamot na nagbababa ng presyon ng dugo na naglalaman ng aliskiren.

Espesyal na mga Pag-iingat

Kausapin ang iyong doktor o parmasyutiko bago uminom ng Irbesartan kung ikaw ay:

Nagkaroon o may labis na pagsusuka o pagtatae habang ginagamot.
Uminom ng "water tablets" (diuretics) o nasa mababang sodium na diyeta.
May problema sa kidney, kabilang ang pagkakaroon ng narrow na kidney artery o may history ng kidney transplant.
May problema sa puso, tulad ng narrowing ng mga heart valves, pinalaking kalamnan ng puso, o heart failure.

Pagpapasuso: Sabihin sa iyong doktor kung ikaw ay nagpapasuso o balak magpasuso. Hindi inirerekomenda ang Irbesartan para sa mga ina na nagpapasuso, at maaaring pumili ang iyong doktor ng ibang paggamot kung nais mong magpasuso, lalo na kung ang iyong sanggol ay bagong panganak o ipinanganak nang maaga.
