  • Dosage for Kids 2–10 years: Dosing is based on body weight, with a loading dose given over 3 divided doses, followed by a maintenance dose given in 2 divided doses daily.

Lower doses are often necessary for older individuals, those with low lean body mass, or those with impaired kidney function. Maximum daily dosage varies greatly and is determined by careful monitoring of blood levels and clinical response.

Contraindications
