Description

Indications/Uses

Used in the management of hypertension, angina pectoris, cardiac arrythmias, myocardial infarction, anxiety disorders and tremor.
 

Dosage/Direction for Use

In hypertension propranolol hydrochloride is given in initial doses of 40 to 80 mg twice daily increased as required to usual range of 160 to 320 mg daily; some patients may require up to 640 mg daily. Propranolol hydrochloride is not suitable for emergency treatment of hypertension.
In angina pectoris, initial doses of propranolol hydrochloride 40 mg is given 2 to 3 times daily are increased as required to a usual range of 120 to 240 mg daily. Some patients may require up to 320 mg daily. Propranolol hydrochloride is administered within 5 to 21 days myocardial infarction in doses of 40 mg given four times for 2 to 3 days followed by 80 mg twice daily. Another regimen is to give 180 to 240 mg daily in divided doses.
Propranolol hydrochloride may be given in doses of 30 to 160 mg daily in divided doses in the long-term management of cardiac arrhythmias. Or as prescribed by the physician.
 

Contraindications

Pregnancy.
 

Special Precautions

Patients with congestive heart failure should be digitalized first before giving Propranolol hydrochloride. Therapy should not be stopped abruptly in angina as precipitation of myocardial infarctions and ventricular arrhythmias may result. Contraindicated to pregnancy.
 

Adverse Reactions

The most frequent adverse effects are related to its beta adrenergic blocking activity. Among the serious adverse effects are heart failure, heart block and bronchospasm. Troublesome subjective side effects include fatigue and coldness of the extremities.
 

Storage

Store at temperatures not exceeding 30°C.
 

Action

Pharmacology: Pharmacokinetics: Propranolol is almost completely absorbed from the gastro-intestinal tract, but is subject to considerable hepatic tissue binding and first-pass metabolism. Peak plasma concentrations occur about 1 to 2 hours after an oral dose. Plasma concentrations vary greatly between individuals. It has high lipid solubility. Propranolol crosses the blood-brain barrier and the placenta. The drug is distributed into breast milk. Propranolol is more than 90% bound to plasma proteins. Its plasma half-life is about 3 to 6 hours.
